Extract commands to updatable .therapy/commands.md

Moves all customization commands (persona, modality, structure, update, import) to a separate file that gets copied to .therapy/ during setup. This allows existing users to receive new commands via the update flow. Co-Authored-By: Claude Opus 4.5 <noreply@anthropic.com>
